Case management has been part of the human service delivery system and in meeting the needs of vulnerable and at-risk populations since the mid- to late 1800's. Support coordination, as an outgrowth of case management, is almost exclusively used to support the IDD/A population and has distinctive features distinct from case management. This evolution has placed support coordination in a category related to but separate and distinct from case management.
