Planning for the transition back into their community for people with Intellectual and Developmental Distinctions from incarceration begins on day one of incarceration and involves a collaborative approach. The Coordinated Community Response model applied to re-entry for people with IDD represents a pathway to long term success. But it is only one leverage point in a complex system.
People who experience Intellectual and Developmental Distinctions and co-occurring chronic challenges to their mental, physical, social and behavioral well-being face at times insurmountable odds when trying to get the appropriate types and levels of care from a system not designed for collaboration or integration.
The ways in which we as a society provide services to human beings in need have come a long way. Our models and thinking about vulnerable people have moved into the systems age, but the systems through which we provide support have remained stuck in the machine age.
